A company is looking for a Community Support Team Qualified
Professional.
Key Responsibilities
Plan, coordinate, and oversee therapeutic and rehabilitative
services for consumers
Develop, implement, and assess the client's Person-Centered Plan
(PCP)
Provide case management, supportive counseling, and skill-building
interventions
Required Qualifications
Master's degree in a human service field with one year of relevant
experience, or
Bachelor's degree in a human service field with two years of
relevant experience
